-Your date of birth
- The type of payment you will provide for your services.
If you intend to utilize insurance please provide a picture of the front and back of your insurance card.
- A brief description of the reasons why you are seeking counseling
- Your availability for appointments
You will receive an email back within 24-48 hours.
